Immunization Information for 12th Graders for the 2022-2023 School Year
Attention Parents students that will be in the 12th Grade for the 2022-2023 school year (current 11th graders):

Below are the requirements from the Ohio Department of Health for school attendance whether in person, online, or virtual. It is required by law, Ohio Revised Code 3313.67 and 3313.671, that students be up to date on immunizations to attend school. All students entering 12th grade are required to be up-to-date with the Meningococcal vaccine. If you choose NOT to immunize then there must be an exemption or
waiver on file for the current school year.

Please do one of the following BEFORE the first day of school 2022-2023:

- If your child has already received these immunizations, please upload an updated record in OneView.

- If you child needs these immunizations, please call your family physician, Erie County Health Department, or Family Health Services for an appointment.

- If you choose not to immunize your child, please complete and return the exemption form

- If you do not have immunizations up-to-date or current records uploaded to OneView at the start of school in the fall, your child risks being excluded from attending school.
