2020 Immunizations at Family Health Services Clinic - October 14th & 28
On October 14th and October 28th the Family Health Services Clinic at Sandusky High School is offering by appointment the following to ALL enrolled Sandusky City Schools students:
    IMMUNIZATIONS,
    PHYSICALS, and/or
    FLU SHOTS.
To schedule your student an appointment at the Sandusky High School Clinic, call 419-984-1042.
Students enrolled in school are required by Ohio law to be up to date for their grade level with immunizations for school attendance even if attending school virtually. Please see the “Immunization Summary for School Attendance”  to check your student’s grade level and immunization record to be certain your student is up to date. If you choose not to immunize, a waiver or exemption must be completed each school year.

Kindergarten, seventh (7th) and twelfth (12th) grade students are typically in need of immunizations before school starts. If you have a student in kindergarten, seventh or twelfth grade, please look at the “Immunization Summary for School Attendance”  to verify your child has received the required immunizations. Additionally, if your student was not up to date in past school years, your student will need immunizations this year.

Immunizations may be obtained at your private family physician, Family Health Services (419-984-1042), the Erie County Health Department (419-626-5623), Kroger Little Clinic, and some pharmacies. Check with your insurance to determine what options are available for your student.

Once your student receives the immunizations, please send a copy directly to your student’s school, or to the Sandusky Board of Education C/O Student Services.
